Registration forms in Badgie: create, share, review and approve
How to create a public registration form in Badgie, share its link or QR code, review each submission with duplicate detection and approve it so the student, their family, classes and subscription are set up automatically.

Quick answer
In Badgie, Forms lets families sign up on their own from a public link or a QR code. Each submission arrives as a request with a reference; the club reviews it (Badgie flags if that person already exists by email, ID number, phone or name and date of birth) and clicking Approve creates or links the student's and guardian's profiles, enrolls them in their classes, activates their subscription if there is one and sends the emails and login details. It also supports per-student questionnaires with no review.

Where to find it
- Web (club): menu Community → Forms (/dashboard/forms). Each form has the Results, Edit form, Settings and Share tabs.
- Public page: badgie.com/en/f/<club>/<form>, in the language the family picks, in light or dark mode.
- AI agent (Badgie Agentic): creates, edits, publishes, duplicates and configures forms and reads their submissions.
The form list
At the top you see how many requests are waiting for your decision and the submissions in the last 30 days. The table shows each form with its Status (draft or published), its Submissions and what is pending, with shortcuts to send, duplicate, test and edit. The Status and Form class filters narrow the list.
Creating a form
- Click + Create form.
- Choose the Form class: Enrolment (asks for identity, is reviewed and creates the student) or Questionnaire (per student, one question per screen, no review).
- Enter the Name and the URL (slug), which becomes the public address.
- Click Create form and the editor opens.
Building the steps in the editor
The editor splits the form into steps (for example: Introduction, Swimmer details, Group and level, Health and kit, Family, Authorizations) with + Add step. On the right you get the Preview exactly as the family will see it.
- Identity fields: first name, last name, date of birth, ID number, email, phone, address, profile photo and ID document (front and back).
- Guardians: first name, last name, email, phone, relationship (father, mother, legal guardian) and the option for the guardian to also be a student.
- Health: allergies, illnesses, medication and injuries.
- Choose your class: the family picks the group and days, with spots left, full or waitlist, and the price.
- Consents and signature: image use, terms and conditions, legal texts and signature.
- Payment: if the registration includes a plan, the family authorizes their card with Stripe; the club charges nothing until it accepts the registration.
Changes are saved as a draft; the public link doesn't change until you click Publish.
Settings
- Approval mode: Manual (review each) or Automatic (conflict-free): in automatic mode only registrations with no matches are approved; doubtful ones stay pending.
- Send email on rejection: tells the family, with the reason.
- Notification emails: who is notified of each new registration.
- Availability: Opens on, Closes on and Max submissions. Outside the window or when full, the public page tells the family.
- For questionnaires, the Questionnaire type (Performance, Survey, Rating or Other) decides where answers land in the student profile.
Sharing the link
Reviewing and approving requests
- Open the form: the Results tab lists the Submissions with reference, name, email, date and status; filter by All, Pending or Rejected, search, sort and Export.
- Click a request: you'll see What was sent, Identities (possible matches with people already in Badgie: same email, ID number, phone or name and date of birth) and On approval (what will happen).
- If there are matches, choose Same person, Link as family or Create new person; Badgie explains the consequence of each option.
- Click Approve or Reject (with a reason that is sent to the family).
After approval, the What the approval did list confirms each step: person created or linked, school enrollment, class enrollment, paid subscription, approval email, subscription welcome email and login details sent. If a class could not be enrolled, Badgie warns you so you can do it by hand from the calendar.
The Summary view groups the answers with a chart per question.
Welcome email
The Welcome email button on the list sets up the automatic email the family receives when a subscription is activated (when a form with a plan is approved or a student is added to a class). You can switch it off or write your own subject and body with the variables {{student}}, {{plan}}, {{price}}, {{school}} and {{class}}; if left empty, Badgie's template with the club logo, plan and price is used.
What families see
The family opens the link or scans the QR code and sees the form step by step with the club's branding, in their language, on phone or computer. Their answers are saved automatically on their device, so they can stop and continue later. If they already have a Badgie account, they can request an email code and fill it in faster with their details. At the end they get a reference: "Registration received" if the club reviews it, or "Registration complete" if it is approved straight away.
By sport
- Swimming and group-based schools: level, group and schedule steps with Choose your class, spots and waitlist.
- Martial arts: the form can ask for the current belt.
- Football, tennis, padel and golf: the same form works for season sign-ups and trials; with a paid plan, the family authorizes the card when registering.
- Questionnaires: ratings, surveys and performance tests per student, landing in their profile.
With the AI agent
The Badgie agent works with real form tools:
- forms_create, forms_edit and forms_clone: create, build or duplicate a form (only the draft is touched).
- forms_publish: publishes it and gets the link ready; it sends nothing to anyone.
- forms_settings: changes the settings (approval, dates, cap, notifications).
- forms_classes: reads the classes offered by the form.
- get_forms and get_form_submissions: list forms and read their submissions.
Example: "Create a registration form for the 2026/27 season with student details, guardian, group and image consent". Approving or rejecting requests is done by the club from Results.
Frequently asked questions
Does the family need an account to register?
No. Opening the link is enough. If they already have an account, they can verify with a code to fill it in faster.
What if the student already exists in Badgie?
The request shows the match under Identities and you choose whether it's the same person, whether to link as family or create a new profile. Nothing is duplicated without your decision (in automatic mode, doubtful ones stay pending).
Can I close the form on a date or when it's full?
Yes: under Settings → Availability, with Closes on and Max submissions.
Does editing the form break the link I already sent?
No. You edit a draft and the link stays the same; changes reach families when you click Publish.
Is the family charged when registering?
If the form includes a plan, the family authorizes their card, but the club charges nothing until it approves the registration.
